„Building an integrated system of care: key factors for success“

Integrated care lies at the heart of the aims of the Health and Social Care Act – to put patients first, improve health outcomes and empower health professionals. Integrated care is a vital component of health and social care reform yet there continues to be a gap between policy intent and practical application.

The business case for integrated care is clearly defined and the vision for a more integrated health and social care system must now be realised at scale and pace. Future challenges in the system can only be overcome by focusing on the health and wellbeing of whole populations and allowing NHS leaders the freedom to innovate and to embed new ways of working over the medium to long term.

But putting models of integrated care into practice continues to pose substantial challenges. This conference will bring together those who have experienced success and have made a difference to individual patient experience and the quality and efficiency of care across whole populations.
