Der DIGILO ist eine digitale Suchmaschine für regionale Gesundheitsangebote, entwickelt von OptiMedis. Die Plattform lässt sich flexibel anpassen und in bestehende Webseiten oder Anwendungen integrieren – ideal für Kommunen, Krankenhäuser, Krankenkassen, Betriebe und andere Organisationen, die die Gesundheitsversorgung in ihrer Region sichtbar machen möchten.

FORTA

Mit FORTA zur richtigen Medikation der Versichertenpopulation

Die Arzneimitteltherapie älterer Menschen ist eine Herausforderung, erst recht, wenn Patienten multimorbide sind und zahlreiche Medikamente parallel einnehmen. Risiken wie schwere Neben- oder Wechselwirkungen müssen vermieden, gleichzeitig Chancen, wie zum Beispiel die Verhinderung von Schlaganfällen, genutzt werden. OptiMedis hat in Kooperation mit Prof. Dr. Martin Wehling und auf Grundlage der FORTA-Klassifikation einen Algorithmus entwickelt, durch den Wirkstoffe in Verbindung mit altersrelevanten Indikationen hierarchisch von A (positiv) bis D (negativ) eingeteilt werden. Er wertet die Arzneimittelversorgung in Bezug auf Unter- und Überversorgung aus – sowohl auf regionaler als auch auf Praxis- und Patientenebene.

Mit dem Algorithmus können wir große Datenmengen auswerten und mit einem Score aus der Summe aus Über- und Untertherapiefehlern pro Patient bewerten. Das Instrument eignet sich zum Beispiel zur Anwendung in der epidemiologischen Forschung zur Beschreibung der Arzneiversorgung älterer Kollektive. Es kann Gruppenvergleiche und Zeitreihenvergleiche für die wichtigsten Kenndaten der Versorgungsqualität liefern. Sie erhalten von uns ein interaktives Dashboard, das sich auch zur Strukturierung von Medikationskonsilen eignet.

Zur Anwendung des Algorithmus gibt es Studien und Auswertungen in Deutschland, Spanien, Frankreich und der Schweiz sowie mehrere Veröffentlichungen.

Mit unserem Online-Tool DOKpit können Ärzt:innen die Qualität der eigenen Leistung schnell und unkompliziert einschätzen und sich mit anderen Praxen im Netz vergleichen. Versorgungslücken schließen, Gesundheitschancen in benachteiligten Stadtteilen verbessern

In einem Gesundheitskiosk werden die Menschen niedrigschwellig in allen Fragen zur Gesundheit und Gesundheitsförderung beraten – und das in möglichst vielen verschiedenen Sprachen, ausgerichtet auf die Bevölkerungsstruktur in den jeweiligen Stadtteilen. Das medizinisch ausgebildete Kioskpersonal bereitet zum Beispiel Arztbesuche vor und nach, aktiviert die Versicherten zu mehr Eigenverantwortung, bietet spezielle Gesundheitskurse und -programme (z. B. für chronisch Kranke) an und vermittelt Hilfe im Stadtteil.

OptiMedis hat das Konzept für den deutschlandweit ersten Gesundheitskiosk im Hamburger Stadtteil Billstedt im Rahmen eines Entwicklungs- und Handlungskonzepts ausgearbeitet und zusammen mit dortigen Ärzt:innen, Gesundheitsakteuren und sozialen Einrichtungen weiterentwickelt.

Patient:innen aktivieren, Integrierte Versorgung koordinieren

Um über eine gesamte Region verteilt qualifizierte Anlaufstellen für erste Fragen rund um die Gesundheit zu etablieren, werden Fachkräfte quer über alle Professionen des Versorgungssystems hinweg für die Weiterqualifizierung rekrutiert – zum Beispiel Medizinische Fachangestellte (MFA) aus Arztpraxen, Pharmazeutisch-Technische Assistent:innen (PTA) aus Apotheken, Pflegekräfte, Therapeut:innen und Fitnesstrainer:innen, Ökotropholog:innen und Hebammen. Diese durchlaufen eine spezielle, durch uns entwickelte Weiterbildung.

Sie finden gemeinsam mit ihren Klient:innen heraus, welche Unterstützung sie brauchen, vereinbaren Gesundheitsziele mit ihnen und vermitteln passende Angebote in der Region. Außerdem sind sie Ansprechpartner:innen für die Teilnahme am besonderen Versorgungsangebot der Gesundheitsnetzwerke und somit eine wichtige Verbindungsstelle zwischen der medizinisch-therapeutischen Versorgung und dem zusätzlichen Gesundheits- und Versorgungsmanagement.

Gemeindepfleger:innen

Die Lücke zwischen medizinischer und psychosozialer Versorgung schließen

Die Gemeindepfleger:innen suchen ältere Menschen in ihrem eigenen Zuhause oder im Pflegeheim auf, verschaffen sich einen Überblick über die Situation und unterstützen sie dabei, bestehende Angebote zu nutzen. Sie planen bedürfnisorientiert alle notwendigen Versorgungs- und Unterstützungsleistungen mit den an der Versorgung Beteiligten und schließen so die Lücke zwischen medizinischer und psychosozialer Versorgung. So fungieren sie als wichtige Schnittstelle zwischen den älteren Menschen und etwa dem Pflegestützpunkt, Pflegediensten und -einrichtungen sowie passenden Diensten und Angeboten. Besonders wichtig ist dabei, dass die Menschen auch über eine längere Zeit begleitet werden. Pflegeleistungen übernehmen die Gemeindepfleger:innen nicht. Ziel ist es, dass die Menschen durch diese Unterstützung möglichst lange gut versorgt zu Hause leben können.

Our healthcare system is not as effective as it could be — a reality that has become even more apparent since the pandemic: physicians, nurses, and therapists face immense time pressures; there is a shortage of young and skilled professionals; many hospitals are fighting for survival; and costs are spiraling out of control. Meanwhile, health promotion and patient engagement remain marginal priorities, digitalisation is progressing slowly, and sectors and stakeholders are poorly connected.

The OptiMedis model

The OptiMedis model for integrated, population-oriented care encompasses the development, implementation, and financing of innovative healthcare networks. Its core focuses are health promotion, coordinated care management, cooperation, and digital innovation. Together with local health and social care stakeholders, we create structures in which all parties involved in healthcare work more closely together and overcome existing boundaries. In May 2023, the OECD recognized the OptiMedis model in their report on international integrated care models.

Quadruple Aim

The model aims to improve the health status of the population, increase patient satisfaction, and enhance job satisfaction among health and social care professionals. At the same time, it helps to curb cost growth.

The right incentives are crucial for quality and efficiency

In the OptiMedis model, incentives are reversed so that keeping people healthy is rewarded. Prevention, health promotion, patient activation, and targeted care management – especially for those with chronic illnesses – are actively promoted, resulting in better health at lower cost.

Innovative financing model: Shared Savings

In our regional networks, we enter into so-called shared-savings contracts with health insurance funds. This means that if we succeed in improving the health of the regional population while at the same time reducing the overall costs for the health insurance funds, the regional healthcare management company, which coordinates local care, receives a share of the savings. After all, when insured individuals are more active and engaged in their own health, and when duplicate tests, drug interactions, and unnecessary hospital stays are avoided, the health insurance funds also incur lower costs.

The digital health navigator

With the digital health navigator DIGILO, we have developed a search engine for integrated healthcare networks that can be used in any region. Since 2020, the platform has been utilized, for example, in the Healthy Werra-Meißner district in Hesse. Interested users can find over 1,400 health services around Eschwege, including physicians, therapists, nutritionists, sports clubs, pharmacies, fitness providers, and many more, enabling them to quickly and easily find the right service nearby. Health navigators and community nurses are also supported by DIGILO in their consultations.

Ensuring optimal medication for the insured population with FORTA: By analyzing the prescriptions of older patients, we can identify both over- and under-treatment, enabling targeted improvements in medication management.
