Equitable healthcare – without soaring expenses

Our healthcare system is not as effective as it could be — a reality that has become even more apparent since the pandemic: physicians, nurses, and therapists face immense time pressures; there is a shortage of young and skilled professionals; many hospitals are fighting for survival; and costs are spiraling out of control. Meanwhile, health promotion and patient engagement remain marginal priorities, digitalisation is progressing slowly, and sectors and stakeholders are poorly connected.

The good news is: better care is already possible today!

Because we already have excellent and specialized providers, strong local social structures, and proven concepts for better, health-promoting care — what’s missing is clever integration, meaningful cooperation, and a clear focus on patient benefit. That is why we develop regional healthcare solutions, up to building entire healthcare networks, connecting medical practices, hospitals, pharmacies, clubs, schools, businesses, and municipalities. At the same time, we reduce the expenditures of the health insurance funds that enter into corresponding contracts with us.

Through evidence-based public health interventions and campaigns, we improve the health of the population — successful examples include our health navigators and the health kiosk concept. We also support hospitals in their transformation processes and develop innovative concepts to make meaningful use of existing structures.

Our goal: modern, cross-sector healthcare that is guided by the needs of local communities and establishes future-proof structures.

The OptiMedis model

The OptiMedis model for integrated, population-oriented care encompasses the development, implementation, and financing of innovative healthcare networks. Its core focuses are health promotion, coordinated care management, cooperation, and digital innovation. Together with local health and social care stakeholders, we create structures in which all parties involved in healthcare work more closely together and overcome existing boundaries. In May 2023, the OECD recognized the OptiMedis model in their report on international integrated care models.

Quadruple Aim

The model aims to improve the health status of the population, increase patient satisfaction, and enhance job satisfaction among health and social care professionals. At the same time, it helps to curb cost growth.

The right incentives are crucial for quality and efficiency

In the OptiMedis model, incentives are reversed so that keeping people healthy is rewarded. Prevention, health promotion, patient activation, and targeted care management – especially for those with chronic illnesses – are actively promoted, resulting in better health at lower cost.

Innovative financing model: Shared Savings

In our regional networks, we enter into so-called shared-savings contracts with health insurance funds. This means that if we succeed in improving the health of the regional population while at the same time reducing the overall costs for the health insurance funds, the regional healthcare management company, which coordinates local care, receives a share of the savings. After all, when insured individuals are more active and engaged in their own health, and when duplicate tests, drug interactions, and unnecessary hospital stays are avoided, the health insurance funds also incur lower costs.

Development and management of innovative healthcare networks

We develop integrated healthcare networks where hospitals, medical practices, nursing care, pharmacies, and social services collaborate across sectors. Our main focuses are health promotion, digital innovations, and care management. Insured individuals are actively involved, costs are reduced, and health benefits for the region are increased.

Strategy and site development for hospitals

From analysis and concept development to implementation – we offer comprehensive strategic consulting. Using regional care data, hospital reports, and expert interviews, we develop innovative concepts to position hospitals and their locations for the future and support them throughout their transformation processes.

REGIONAL ANALYSES

From the analysis of regional healthcare structures and needs to location, service, market, and cost-effectiveness analyses: with the help of our reliable data insights, you can gain a comprehensive overview of healthcare delivery in a region, as well as for specific subpopulations, individual practices, or particular topics. This enables you to make priority decisions, plan targeted interventions, and adjust them as needed.

HEALTH & HEALTHCARE MANAGEMENT

Based on regional needs, we develop healthcare programmes (e.g., for people with chronic conditions), establish effective quality and medication management, expand workplace health promotion, and increase health literacy and activation in the population through health kiosks, health navigators, and community health workers.

PROJECT INITIATION & FUNDING ACQUISITION

We support the design of healthcare projects and identify and acquire funding and project resources (e.g., through the German Innovation Fund). We have extensive contacts with potential cooperation partners and payers and handle contract development.

To do this, we test health apps, online interventions, disease-specific programmes, and much more in real-world care settings, evaluating their benefits, acceptance, and feasibility. DIGILO search engine –
The digital health navigator

With the digital health navigator DIGILO, we have developed a search engine for integrated healthcare networks that can be used in any region. Since 2020, the platform has been utilized, for example, in the Healthy Werra-Meißner district in Hesse. Interested users can find over 1,400 health services around Eschwege, including physicians, therapists, nutritionists, sports clubs, pharmacies, fitness providers, and many more, enabling them to quickly and easily find the right service nearby. Health navigators and community nurses are also supported by DIGILO in their consultations.

Ensuring optimal medication for the insured population with FORTA: By analyzing the prescriptions of older patients, we can identify both over- and under-treatment, enabling targeted improvements in medication management.
